Internal bleeding may result from a direct blow to the body, from strains, and from diseases such as peptic ulcer.

Internal bleeding can be concealed or visible. Bleeding round a broken limb may be concealed but may be detectable because it causes a swelling, the size of which shows the amount of the bleeding. Bleeding into the chest or abdominal cavities may be revealed if blood is coughed up or is vomited. Stab and puncture wounds can cause serious internal bleeding.

The casualty will be shocked. At first he will be pale, giddy, faint, and sweating. His pulse rate and respiration rate will rise. Later his skin will become cold and his extremities will become slightly blue. The pulse will become difficult to feel and very rapid. The breathing will be very shallow. He will complain of thirst and nausea, become restless and complain that he cannot breathe properly. These three signs show that the bleeding is still occurring. Later he will cease to complain, lose interest in his surroundings and become unconsciousness.

The most important indication of continuing bleeding is a rising pulse and falling blood pressure. Anyone in whom internal bleeding is suspected must therefore have his pulse rate and blood pressure recorded at fixed and frequent intervals, say 5-10 minutes. After about an hour of such recording it should be clear whether or not he is bleeding internally. If the patient’s blood pressure remains about normal, and the pulse rate falls or remains steady, he is not bleeding.

Сильный порез или удар очень опасны из-за возможной большой потери крови. Кровотечение из раны в виде струи алой крови смертельно опасно для пострадавшего и возникает при повреждении артерии. Для его остановки:

- немедленно прижмите артерию у того края раны, который расположен ближе к сердцу. При расположении раны на руке или ноге, не отпуская пальца, сдавливающего артерию, поднимите конечность, наложите жгут (tourniquet) выше раны и только после этого прекратите пальцевое нажатие. Не накладывайте жгут слишком туго. Жгут нельзя держать более 2-х часов. После этого срока прижмите артерию пальцем и распустите жгут, через 5-10 минут вновь затяните жгут.

Все остальные виды кровотечений (кроме артериального) останавливаются давящей стерильной повязкой, наложенной на рану. Простейший и наиболее эффективный метод остановки кровотечения – это прижать тампон или использовать что-нибудь вместо него, если нет ничего, можно использовать руку или палец.
